Alzheimer's & Memory Care Rates
At Autumn Leaves we care for your loved one like family. Our secure, home-like communities provide all the comforts of home in a safe, comfortable atmosphere. Our Company policy is to provide the highest quality care at a competitive, flat monthly rate.

At Autumn Leaves there are no levels of care or periodic assessments to adjust your monthly rate.

 When you complete an Admission Agreement your rate is locked in, regardless of the varying needs of your loved one, and includes:

  • On-site nurses with specialized expertise in dementia care
  • Caregivers trained in memory care who complete ongoing, monthly inservice training
  • On-site dietary staff that prepares 3 nutritious, fresh meals each day
  • Between-meal snacks available 24-hours a day
  • Individualized care plan developed in cooperation with your physician
  • Life Enrichment Coordinators who create group and individual activity programs especially suited for individuals living with Alzheimer's disease and memory impairment
  • Assistance with Activities of Daily Living (ADL's) including bathing, dressing, toileting, feeding and mobility
  • Medication management
  • Incontinence care
  • Health monitoring (vital signs, lab tests, therapy needs, insulin levels)
  • Daily housekeeping
  • Laundry (personal laundry and linens gathered, laundered, folded and put away)

Each of our residents and their families have unique financial and payment needs and we work with each one to assist them in finding the best plan for their family. We are committed to helping families and their loved ones work within their budgets and encourage them to develop a financial plan for their loved one’s stay in our communities. We work with a number of financial planners, veterans benefits administrators and educators and long-term insurance representatives to make sure our families know all payment options available to them.

Autumn Leaves accepts a combination of funds including:

  • Private pay out-of-pocket including savings plan annuities, life insurance policies or reverse mortgages
  • Private long-term care insurance
