Jane and Win Warren started Constant Care Corporation to be a company that offered a family approach to caregiving - A company where ownership plays an important role in daily operations. Constant Care developed Autumn Leaves to bring their management approach to individuals with Alzheimer's disease.

Autumn Leaves was created specifically to accommodate residents with memory impairment. This design allows individuals to live in a residential environment that provides security and a more nurturing lifestyle. An uplifting, supportive environment allows residents to continue their lives as independently as possible with just the right amount of assistance.
